If Your Card has Been Lost or Stolen, or You Suspect You’ve Been a Victim of Fraud, Request a New Card.

For the fastest and most secure processing, Members can now:
1. Request a replacement debit card directly through Online & Mobile Banking, with immediate digital access to your new card. Experience the convenience of Digital Debit Cards with secure, immediate access right in your pocket.
If your debit card is lost or stolen, you will get a new card number, expiration date, and CVV, and the old card will be blocked for your protection. If your card is damaged, you will keep the same card number but receive a new expiration date and CVV.
As soon as your request is processed, you can instantly add your debit card to your Apple Wallet, Google Wallet, Samsung Wallet, OR other supported digital wallet. Then, you can start using it right away!
2. OR you can log into the Mobile App or Mobile Banking and fill out the Digital Debit Card Request Form.
Alternatively, you can complete and return the fillable Debit Card Request Application to Member Services. Please be sure to print, sign, and submit the completed form along with a copy of your identification. You may provide this form via email to Member Services at mservices@actorsfcu.com or by visiting one of our branch offices.
